Registered Nurse- Hospice Admissions Leader

We are looking for a compassionate and detail-oriented Admissions Registered Nurse Leader to be an integral part of our dedicated healthcare team. In this critical role, you will serve as the first point of contact for new patients, ensuring their transition into care is smooth, efficient, and compassionate. Your expertise in nursing assessments, patient history documentation, and care coordination will be instrumental in delivering high-quality, patient-centered care, especially within the senior and long-term care settings.

Key Responsibilities

• Conduct Thorough Assessments: Perform comprehensive evaluations of incoming patients, gathering detailed medical histories and assessing their current health status to ensure a seamless transition into care.
• Collaborate with Healthcare Teams: Work closely with interdisciplinary teams to create individualized care plans that address each patient’s specific needs, promoting the highest standard of care.
• Support Patients and Families: Provide education and emotional support to patients and their families, explaining admission procedures, care options, and next steps in their healthcare journey.
• Ensure Regulatory Compliance: Maintain compliance with all relevant regulations, ensuring accurate and complete patient documentation in line with healthcare standards and guidelines.
• Leverage Nursing Expertise: Utilize your knowledge of physiology, basic life support, and sterile processing during patient assessments to ensure optimal care practices are followed.
• Assist with Coding and Documentation: Accurately assist in coding patient records for billing and insurance purposes, ensuring all documentation reflects the patient's care needs and treatments.
• Support Special Care Needs: Apply specialized nursing techniques when working with patients with developmental disabilities, tube feeding needs, or other unique conditions to provide personalized, empathetic care.
• Monitor Changes in Condition: Stay alert to any changes in patient condition during the admission process, including medical needs related to tube feeding, ophthalmology, or other health concerns.

Qualifications

• Registered Nurse (RN) licensure in good standing.
• Previous experience in senior care or long-term care settings is strongly preferred.
• Solid understanding of physiology, basic life support (BLS) techniques, and sterile processing.
• Familiarity with nursing documentation coding practices is beneficial.
• Experience working with individuals with developmental disabilities is a plus.
• Exposure to family planning concepts is advantageous.
• Proficiency in tube feeding procedures is desirable.
• Strong communication skills and the ability to work effectively within an interdisciplinary healthcare team.

Pay: $115,000.00 - $130,000.00 per year
