Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Long-Term Care (LTC) / Skilled Nursing team. In this essential role, you will deliver high-quality, patient-centered nursing care to residents with chronic conditions, complex medical needs, and varying levels of functional ability. You will play a key role in promoting resident safety, comfort, dignity, and overall well-being while collaborating closely with interdisciplinary care teams. Duties and Responsibilities
- Conduct comprehensive resident assessments, including vital signs, physical examinations, and detailed health histories using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Administer medications, treatments, and injections safely and accurately while adhering to aseptic techniques, infection control protocols, and HIPAA regulations.
- Provide skilled nursing care for residents with chronic illnesses, post-acute recovery needs, cognitive impairments, and mobility limitations.
- Manage and monitor complex clinical interventions including wound care, IV therapy, catheter care, enteral feedings (G-tubes), ostomy care, and dialysis coordination.
- Provide care for residents requiring tracheostomy and ventilator support; experience with vent/trach management is a strong plus.
- Coordinate resident care through case management and care planning in collaboration with physicians, therapists, social workers, and families.
- Monitor changes in resident condition, recognize early signs of deterioration, and initiate appropriate interventions or escalation of care.
- Respond to medical emergencies within the facility, including falls, respiratory distress, and acute changes in condition.
- Educate residents and families on disease management, medication adherence, nutrition, and long-term care planning.
- Ensure accurate and timely documentation in compliance with regulatory standards, including NCQA guidelines and ICD-9/ICD-10 coding requirements.
- Support quality improvement initiatives and maintain compliance with state and federal long-term care regulations.
Qualifications
- Current RN or LPN license in good standing with the state nursing board.
- Experience in long-term care, skilled nursing facilities, post-acute care, rehabilitation, or assisted living preferred.
- Strong clinical knowledge of anatomy, physiology, pharmacology, and medical terminology.
- Proficiency with EMR/EHR systems (PCC).
- Experience with complex resident care including wound management, tube feedings, ostomy care, and chronic disease management.
- Ventilator and tracheostomy care experience is a plus, but not required.
- Ability to work collaboratively within an interdisciplinary healthcare team.
- Excellent communication and interpersonal skills for engaging residents, families, and healthcare professionals.
