The Opportunity
We are seeking a skilled, experienced, and compassionate Registered Nurse to serve in a hybrid Lead RN / Nurse Manager role within our residential treatment program. This position blends hands-on clinical care with nursing leadership and supervision, offering a unique opportunity to remain deeply connected to patients while also shaping nursing practice, standards, and culture.
In this role, you will function as a lead floor RN while also overseeing and supporting the nursing team. You will play a critical role in ensuring safe, consistent, and high-quality nursing care for clients with psychiatric, medical, substance use, and co-occurring needs at our 2 residential and 1 outpatient site in Temecula.
Working in close partnership with the Medical Director, Clinical Leadership, and Operations, you will help build a nursing model that is structured, collaborative, and deeply human — one that reflects Terra’s commitment to both clinical excellence and employee-centered leadership.

What You’ll Do

Direct Clinical Care & Nursing Practice
· Provide skilled, trauma-informed nursing care including assessments, ongoing evaluations, and treatment plans for clients with Mental Health, SUD, and Co-Occurring conditions.
· Support safe detoxification/withdrawal management protocols (as applicable) under physician/psychiatrist/NP guidance.
· Administer medications with precision, following Terra’s safety protocols and regulatory requirements.
· Monitor for medical or psychiatric changes in condition and intervene promptly and appropriately.
Safety, Risk Management & Crises
· Respond to medical and psychiatric emergencies with strong clinical judgment, calm, and clarity.
· Support the milieu through de-escalation, crisis intervention, therapeutic communication, and safety planning.
· Ensure compliance with all medication, storage, infection control, and nursing standards.
Collaboration & Care Coordination
· As a key member of the interdisciplinary team, collaborate closely with therapists, medical providers, case managers, and behavioral health technicians to provide cohesive, integrated care.
· Participate in clinical rounds and interdisciplinary discussions to support client progress, safety, and outcomes.
· Communicate clearly and compassionately with clients, families, and team members.
· As needed driving duties
Documentation & Compliance
· Complete timely, accurate nursing notes, assessments, and MAR documentation that meet DHCS, Joint Commission, and Terra standards.
· Bring a disciplined, detail-oriented approach to every aspect of nursing practice.
· Follow all scope-of-practice guidelines, medication protocols, and regulatory expectations.
Leadership
· Hire, train, onboard, mentor and supervise nursing team.
· Lead scheduling to ensure appropriate staffing at the facility.
· Oversee nursing policies, procedures, and practices ensure consistency of care, client experience, and regulatory compliance.
Culture & Milieu Management
· Help create a therapeutic environment where clients feel safe and staff feel supported.
· Ensure nursing department operates effectively providing great care & exceptional nursing services.

What You Bring
· Active California RN License
· Valid Driver’s License
· CPR certified or willing to complete CPR certification prior to hire
· 2+ years experience working in residential, inpatient, or structured mental health and substances use treatment settings
· 1+ year nursing leadership experience
· Strong patient-centered assessment, treatment planning, and crisis response skills
· Ability to remain grounded, collaborative, and client-centered in fast-paced or emergent situations.
