Position Summary
The PACT Care Manager Registered Nurse is a core member of the Patient Aligned Care Team, responsible for coordinating and delivering comprehensive, patient‑centered care to Veterans with acute and chronic medical, surgical, and mental health conditions in the outpatient setting. The RN uses focused assessment, critical reasoning, and sound clinical judgment to manage complex and potentially life‑threatening conditions and behavioral health emergencies, while promoting health, preventing illness, and optimizing self‑management.
The PACT RN is a proficient nurse who provides leadership in the delivery of holistic care, collaborates closely with interdisciplinary team members, and leads practice improvements driven by evidence and data. The RN facilitates continuity of care across the continuum, including clinic, diagnostic and procedural areas, Telehealth environments, and care coordination programs.
Responsibilities
- Provide coordinated, evidence‑based nursing care for a panel of Veterans across the lifespan using the full nursing process (assessment, diagnosis, planning, implementation, evaluation).
- Perform focused assessments and exercise expert clinical judgment to rapidly identify, triage, and address complex or emergent clinical and behavioral health needs.
- Lead and participate in interdisciplinary PACT huddles, case conferences, and care planning to ensure safe, timely, and Veteran‑driven care.
- Support care transitions by arranging follow‑up, referrals, education, and community resources across outpatient, specialty, and telehealth settings.
- Provide health education and coaching tailored to Veteran goals, values, culture, and preferences, promoting self‑management and shared decision making.
- Act as an advocate for Veterans, families, and caregivers, addressing care coordination issues, ethical concerns, and barriers to access or quality.
- Utilize VA electronic health records and related systems to document care accurately and timely, track outcomes, and communicate with the team.
- Lead or actively participate in quality improvement initiatives, using evidence‑based practice and data to improve safety, processes, and patient outcomes.
- Mentor, precept, and/or teach other nurses and learners, supporting professional development within the practice setting.
- Demonstrate exemplary customer service, communication, and teamwork with Veterans, families, and all VA staff while supporting VA’s mission and I CARE values.
Basic Requirements
- U.S. citizenship.
- Current, full, active, and unrestricted RN license in any U.S. State, Territory, Commonwealth, or the District of Columbia.
- Graduate of an accredited school of professional nursing (NLNAC/ACEN or CCNE, or as otherwise approved by VA).
- Meets all basic requirements in VA Handbook 5005, Part II, Appendix G6.
Pay: $38.26 - $46.08 per hour
