Position Summary

We are seeking a detail-oriented and proactive Registered Nurse (RN) to oversee medication management, clinical documentation, and ongoing health monitoring for individuals in our DDA Supported Living homes.

This role combines field visits, clinical oversight, and coordination with healthcare providers to ensure the safety and well-being of individuals served.

Key Responsibilities

Medication Management & Compliance

• Conduct routine audits of Medication Administration Records (MARs) in supported living homes
• Verify accuracy of medications and ensure proper documentation
• Update medication changes in individual charts and systems (including PHS)
• Review and verify all prescriptions following medical appointments
• Serve as the primary reviewer of medication orders (scripts) prior to implementation

Clinical Oversight

• Complete DDA admission packets for new individuals and annual renewals
• Monitor overall health status of individuals, including:
• Skin integrity (especially wheelchair-bound individuals)
• Signs/symptoms of urinary tract issues
• General wellness and safety checks
• Ensure proper follow-up care and documentation

Diabetes Management

• Monitor individuals with diabetes and review blood sugar logs weekly
• Communicate with Vanderbilt medical providers regarding insulin adjustments
• Submit blood sugar reports and implement provider recommendations

Coordination & Communication

• Collaborate with Program Coordinators, DSP staff, and leadership
• Provide guidance and oversight on health-related concerns
• Ensure compliance with all DDA and agency policies

Qualifications

• Active Registered Nurse (RN) license in the state of Tennessee
• Experience with DDA/DIDD or IDD populations REQUIRED
• Excellent written and computer skills required
• Experience in home health or community-based services strongly preferred
• Strong knowledge of medication administration and MAR systems
• Experience with diabetes management and insulin protocols preferred
• Ability to travel between multiple residential homes
• Strong attention to detail, organization, and documentation skills
