We are seeking a Registered Nurse Trainer to oversee the healthcare needs of individuals in Community Integrated Living Arrangements (CILA). The RN Trainer will provide medical oversight, staff training, and ensure compliance with healthcare regulations. This role includes on-call responsibilities to address urgent medical concerns, oversight of medication updates and discontinuations, and conducting fall assessments and HRST reviews.
Responsibilities
- Serve as the primary care nurse for assigned CILA homes.
- Conduct assessments and respond to emergency situations.
- Train direct support professionals (DSPs) on medication administration and health protocols.
- Monitor medication records, orders, and administration.
- Review monthly health summaries, vital signs, and medical treatments.
- Coordinate annual physicals, dental exams, and specialist visits.
- Address medication errors, retrain staff, and report incidents.
- Assist with admissions, discharges, and transfers of residents.
- Maintain compliance with Illinois healthcare regulations.
- On-call duties: Provide after-hours support for urgent medical concerns, including medication issues, health emergencies, and staff guidance.
- Medication updates and discontinuations:
- Review and implement medication changes as prescribed by healthcare providers.
- Ensure proper documentation of medication updates and discontinuations.
- Communicate medication changes to staff and provide necessary training.
- Monitor residents for adverse reactions or side effects following medication adjustments.
- Coordinate with pharmacies and physicians to ensure timely medication updates.
- Fall Assessments:
- Conduct fall risk evaluations for residents.
- Implement fall prevention strategies and staff training.
- Monitor and document fall incidents, ensuring appropriate interventions.
- Collaborate with physical therapists and healthcare providers to reduce fall risks.
- HRST Reviews:
- Perform Health Risk Screening Tool (HRST) assessments to evaluate resident health risks.
- Develop care plans based on HRST findings.
- Ensure compliance with HRST documentation and reporting.
- Provide staff training on HRST protocols and risk mitigation.
Qualifications
- Active RN license in the State of Illinois.
- Minimum 1 year of clinical nursing experience (preferably in developmental disabilities).
- Ability to complete RN Trainer certification within 3 months.
- Strong communication, organizational, and leadership skills.
- Experience with electronic medical records (EMR).
- Valid Illinois driver’s license.
- Availability for on-call rotation as needed.

Pay: $72,000.00 - $82,000.00 per year
