Collaborates with the inpatient and/or outpatient interdisciplinary teams to identify patients most likely to benefit from care coordination services.

Assesses the patient’s risk factors and needs for care coordination, clinical utilization management services, discharge planning and preventative services.

Periodically meets with the patient and/or family in to ensure continuity across the continuum of care.

Acts as a liaison and resource in collaboration with physicians and their office staff, hospitalists, care facilities, ancillary providers, health plan case managers and internal departments.

Provides follow-up with patient/family when patient transitions from one setting to another.

Develops effective working relationships with providers, health center leadership and support staff to ensure the needs of the care team are being successfully met.

Analyzes and provides recommendations for ways to improve customer service, improve patient flow, clinical outcomes, increase productivity, and improve utilization of resources.

Participates in quality improvement activities

Adheres to all HIPPA,OSHA, state, other regulatory agencies and NVHC lab manual policy and procedures requirements

Triages phone calls and walk-ins at the clinic where applicable: performing

Draws blood, performs in office lab procedures and prepares specimen for lab processing

Graduate of an accredited nursing school required, Bachelor’s degree in Nursing is preferred
-
Licensed as a Registered Nurse in the State of Nevada is required
-
1-3 years clinical experience in ambulatory setting is preferred

BLS/CPR Certification
-
English/Spanish fluency
-
Knowledge of the essential functions, practices, and procedures of medical clinic and acute care hospital

Demonstrated proficiency with computer programs and data entry, experience with Electronic Health Records highly preferred

Experience in a healthcare setting with diverse populations
-
Ability to communicate via written and verbal communication
-
Ability to exercise good judgement in appraising situations and responding appropriately to underserved population groups.
