Registered Nurse (RN)

The Floor/Cart Registered Nurse (RN) is responsible for providing direct nursing care to residents in a skilled nursing facility, ensuring safe medication administration, accurate documentation, and adherence to Pennsylvania Nurse Practice Act regulations. The RN functions as a clinical leader on the unit, supervising LPNs and CNAs while promoting high-quality, resident-centered care.

Essential Duties and Responsibilities

Clinical Care

  • Perform comprehensive nursing assessments and ongoing evaluations of resident conditions

  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team

  • Administer medications and treatments accurately using the medication cart (“cart nurse” duties)

  • Monitor residents for changes in condition and promptly notify physicians and supervisors

  • Provide wound care, tube feedings, IV therapy (if applicable), and other skilled nursing services

Medication Management

  • Safely prepare, administer, and document medications per physician orders and facility policy

  • Ensure compliance with the “Five Rights” of medication administration

  • Monitor for adverse drug reactions and medication effectiveness

  • Maintain proper storage, security, and inventory of medications

Supervision & Leadership

  • Supervise and support LPNs and CNAs on the floor

  • Delegate nursing tasks appropriately based on scope of practice

  • Provide guidance, coaching, and real-time clinical support to staff

  • Participate in shift reports and ensure continuity of care

Documentation & Compliance

  • Accurately document assessments, medications, treatments, and resident responses in the medical record

  • Ensure compliance with federal, state (PA DOH), and facility regulations

  • Participate in audits, care plan meetings, and quality assurance activities

Communication

  • Communicate effectively with physicians, families, residents, and interdisciplinary team members

  • Respond to resident and family concerns professionally and compassionately

  • Provide resident and family education as appropriate

Safety & Infection Control

  • Follow infection control policies, including standard and transmission-based precautions

  • Ensure a safe environment for residents, visitors, and staff

  • Report incidents, accidents, and variances per facility policy

Qualifications

Required

  • Active Registered Nurse (RN) license in the state of Pennsylvania

  • Graduate of an accredited nursing program

  • Current CPR/BLS certification

  • Ability to work effectively in a fast-paced SNF environment
