Registered Nurse (RN)

Overview
We are seeking an experienced Registered Nurse to join our team as a Per Diem Assessment Coordinator. The successful candidate will conduct comprehensive assessments of patients to determine their eligibility for home health service

• Conduct comprehensive assessments of the patient’s health status and needs.
• Develop and implement individualized care plans based on patient assessments

• *Key Responsibilities*1. Conduct in-home assessments of patients to determine eligibility for home health services.

• 2. Perform comprehensive assessments, including medical history, physical examination, and review of medical records.

• 3. Develop and implement individualized patient care plans.

• 4. Collaborate with physicians, therapists, and other healthcare professionals to ensure coordination of care.

• 5. Communicate assessment findings and patient care plans to healthcare team members.

• 6. Document patient information accurately and efficiently.

• 7. Provide patient education and support.

• Qualifications of the RN supervisor include:

1. A current Georgia RN license to practice nursing and render services by the Georgia

2. Experience in home health services or a related field, experience working with older populations is preferred

3. Knowledge of current methods of home management, the provision of personal care, the ability to supervise and coordinate the work of others, to evaluate situations and make decisions, and to communicate and deal effectively with others

*Preferred Qualifications*

1. Experience with electronic medical records (EMRs)
2. Strong knowledge of Medicaid guidelines and regulations

*Work Schedule*

1. Per diem position (as needed)
2. Flexible scheduling, including weekdays and weekends

Experience

• Current Registered Nurse (RN) license in good standing.
• Familiarity with medical documentation practices and electronic health record system
• Ability to lift patients safely and assist with mobility as needed.
• Strong communication skills and the ability to work effectively under pressure.

Pay: $55.00 per hour
