We are seeking a compassionate and skilled Primary Care Nurse/Registered Nurse (RN) to provide high‑quality primary nursing care to patients in their homes. This role focuses on assessment, care planning, patient education, and coordination of services under a physician’s plan of treatment.

What You'll Do:  

• Complete initial and ongoing patient assessments and develop individualized plans of care
• Provide skilled nursing services, including preventative and rehabilitative care
• Monitor patient progress and respond promptly to changes in condition
• Educate and support patients and families
• Coordinate care with physicians, caregivers, and home health team members
• Maintain accurate and timely clinical documentation
• Supervise and support home health aides as needed

What You'll Need:  

• Active unencumbered MA Registered Nurse (RN) license 
• Graduate of an accredited school of nursing
• Minimum 1 year of RN nursing experience
• CPR certification 
• IV therapy experience a plus
