Position Overview
We are seeking a compassionate, organized, and clinically skilled Registered Nurse (RN) to join our Home Health team. The RN will provide skilled nursing care to patients in their homes, coordinate care with physicians and interdisciplinary teams, and ensure high-quality patient outcomes while maintaining compliance with all regulatory and agency standards.
This position requires strong clinical judgment, excellent communication skills, and the ability to work independently in a fast-paced home health environment.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Perform Start of Care (SOC), resumption of care, recertification, discharge, and routine skilled nursing visits
- Administer medications, wound care, injections, catheter care, IV therapy, and other skilled nursing services as ordered
- Monitor patient condition and report changes to physicians and clinical supervisors
- Educate patients and caregivers regarding disease management, medications, safety, and home care procedures
- Coordinate care with physicians, therapists, case managers, and interdisciplinary team members
- Complete accurate and timely electronic documentation in accordance with agency, state, federal, and Medicare guidelines
- Participate in case conferences and quality assurance activities
- Ensure compliance with HIPAA, infection control, and home health regulations
- Maintain productivity standards and visit schedules
Qualifications
- Active California Registered Nurse (RN) license in good standing
- Minimum of one (1) year nursing experience preferred
- Home health experience strongly preferred
- Knowledge of Medicare, OASIS, and home health documentation preferred
- Current BLS/CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Strong assessment, organizational, and communication skills
- Ability to work independently and manage time effectively
