We are currently seeking a compassionate and skilled Registered Nurse (RN) to support patients in the CLASS (Community Living Assistance and Support Services) program.

Position Overview

This is a flexible, hybrid role that combines remote work with in-home patient visits. The RN will be responsible for conducting assessments, completing recertifications, and developing individualized care plans to ensure high-quality patient outcomes.

Key Responsibilities

• Conduct initial and ongoing patient assessments in home settings

• Complete recertifications in accordance with program requirements

• Develop and update individualized care plans

• Collaborate with interdisciplinary teams, caregivers, and families

• Maintain accurate and timely documentation

• Ensure compliance with all state and program regulations

Qualifications

• Active and unrestricted Registered Nurse (RN) license

• Minimum 1 year of nursing experience (home health or community-based experience preferred)

• Strong clinical judgment and critical thinking skills

• Excellent communication and interpersonal abilities

• Ability to work independently and manage a flexible schedule

• Reliable transportation for in-home visits

Preferred:

• Bilingual (English/Spanish)
