Job Summary
The Registered Nurse (RN) Home Health Director is responsible for the overall clinical oversight, direction, and evaluation of home health services provided by the agency. This position ensures compliance with Arizona Department of Health Services (AZDHS) regulations under Article 12, and all applicable federal and state laws. The RN Director also serves as a member of the Advisory Group, providing clinical expertise, policy guidance, and quality improvement recommendations.
Minimum Qualifications
- Must be a Registered Nurse (RN) licensed in the State of Arizona in good standing
- Must have at least three (3) years of nursing experience
- Must have a minimum of 24 months of experience as a Registered Nurse providing home health services
- Must have knowledge of home health regulations, standards of care, and patient rights
- Must have supervisory or leadership experience (preferred)
Licensure & Certification Requirements
- Current Arizona RN License (unencumbered)
- Current CPR/BLS Certification
- Fingerprint Clearance Card (per Arizona requirements)
- Valid Arizona Driver’s License
Essential Duties and ResponsibilitiesClinical Oversight & Administration
- Direct and supervise all home health services provided by the agency
- Ensure services are delivered in accordance with physician orders, individualized care plans, and regulatory requirements
- Oversee the development, implementation, and review of patient care policies and procedures
- Ensure continuity and quality of care for all members/clients
Regulatory Compliance (AZDHS Article 12)
- Ensure agency compliance with Arizona Administrative Code – Article 12 (Home Health Agencies)
- Maintain readiness for AZDHS inspections and audits
- Ensure proper documentation, recordkeeping, and confidentiality of medical records
- Implement corrective actions for deficiencies identified during surveys
Staff Supervision & Training
- Supervise and evaluate nursing staff and direct care personnel
- Ensure staff are properly trained, competent, and operating within scope of practice
- Provide ongoing education, competency validation, and performance evaluations
- Participate in hiring, discipline, and termination processes
Care Coordination
- Coordinate services with physicians, therapists, case managers, and other providers
- Participate in care planning and case conferences
- Ensure timely initiation of services and reassessment of care needs
Quality Assurance & Performance Improvement (QAPI)
- Develop and implement quality assurance and performance improvement programs
- Monitor patient outcomes and satisfaction
- Review incident reports, grievances, and complaints
- Implement improvements based on data and trends
Pay: $45.00 - $50.00 per hour
