Job Overview

We are seeking a compassionate and reliable Registered Nurse (RN) to join our growing home health and hospice team. In this role, you will provide skilled nursing care to patients in their homes, focusing on comfort, safety, and quality of life.

Responsibilities

• Conduct comprehensive patient assessments, including vital signs and health status evaluations
• Develop and implement individualized care plans in accordance with physician orders
• Provide skilled nursing care including medication administration, wound care, IV therapy, and patient monitoring
• Deliver hospice-focused care including pain and symptom management
• Educate patients and families on medications, disease management, and care plans
• Maintain accurate and timely documentation in EMR/EHR systems
• Collaborate with physicians, case managers, and interdisciplinary team members
• Respond to patient needs and provide timely clinical support

Requirements

• Active Registered Nurse (RN) license in Arizona (in good standing)
• Experience in home health or hospice preferred (new grads with strong clinical skills may be considered)
• Strong clinical skills including wound care, IV administration, and patient assessment
• Ability to work independently and manage time effectively
• Excellent communication and interpersonal skills
• Current BLS certification (ACLS preferred but not required)
• Reliable transportation and valid driver’s license

Preferred Qualifications

• Experience with WellSky (Training available)
• Experience with geriatric or palliative care patients
• IV therapy and wound care experience

Job Types: Full-time, Part-time, Per diem

Pay: $30.00 - $50.00 per hour
