What You’ll Do

• Conduct one-on-one patient care and education in the home setting
• Perform patient assessments and collaborate with the care team on plans of care
• Maintain accurate, timely documentation of patient visits
• Communicate regularly with clinical management and care teams

Requirements:

What We’re Looking For

• Graduate of an accredited RN program
• Current Georgia RN license
• CPR certification
• Reliable transportation, valid driver’s license, and insurance
• Excellent communication, organization, and time-management skills
• Initiative, attention to detail, and ability to multi-task
• Home health / OASIS experience preferred but not required – we provide training!
