Registered Nurse (RN) – Home Health (Remote / Hybrid)

Job Overview

Swan Garden Home Services, LLC is seeking a dedicated Registered Nurse (RN) to join our team in a remote / hybrid role supporting individuals receiving home and community-based services. This position allows nurses to perform many responsibilities from home, including documentation review, care coordination, and staff consultation, while attending required onsite meetings, assessments, and trainings as needed.

This role is ideal for an RN who values flexibility, leadership, and clinical oversight within a community-based care environment.

Swan Garden Home Services, LLC is an Indiana Medicaid Waiver provider serving individuals with intellectual and developmental disabilities and aging adults throughout the Indianapolis and surrounding communities. Our agency provides services under the Community Integration and Habilitation (CIH) Waiver, Family Supports Waiver (FSW), and PathWays for Aging Waiver.

Responsibilities

• Conduct remote health assessments and chart reviews
• Provide clinical oversight and nursing consultation to Direct Support Professionals (DSPs)
• Review and assist with Individualized Service Plans (ISPs) and health support plans
• Monitor medication administration protocols and health documentation
• Participate in incident reviews related to medical concerns
• Ensure compliance with Indiana Medicaid Waiver and BDS standards
• Communicate with physicians, service coordinators, families, and interdisciplinary teams
• Provide training and guidance to staff regarding health-related care
• Attend required onsite meetings, client assessments, and staff trainings

Qualifications

• Active Registered Nurse (RN) license in the State of Indiana
• Minimum 3 years of nursing experience
• Experience in home health, community-based services, developmental disabilities, or case management preferred
• Strong clinical assessment and documentation skills
• Comfortable working independently in a remote environment
• Ability to attend onsite meetings in the Plainfield / Indianapolis area when required
• Reliable transportation for occasional onsite visits

Compensation & Benefits

• $35.53 – $52.85 per hour
