Are you a Registered Nurse (RN) who loves variety and values a healthy work-life balance? We are looking for a dynamic RN to fill a unique "3-2 split" hybrid role. This position perfectly balances hands-on community care with clinical administrative strategy, ensuring your days are never monotonous.

Key Responsibilities

3 Days in the Field (Community Assessments):

• Conduct comprehensive Assessments and Reassessments for patients across Queens.
• Visit patients in their homes to evaluate health status, physical safety, and overall care needs.
• Ensure patients are receiving the appropriate level of support and resources.

2 Days in the Office (Queens Village HQ):

• Work from our headquarters located at 220-05 Jamaica Ave, Queens Village, NY.
• Manage clinical administrative tasks and lead Quality Assurance (QA) initiatives.
• Support daily office operations and collaborate with the internal care coordination team.
• Support orientation and new onboarding services for new hires.

Qualifications

• Valid and unrestricted Registered Nurse (RN) license in the State of New York.
• Prior experience in home health care, community health, or conducting patient assessments is preferred.
• Strong clinical judgment, documentation skills, and attention to detail (Quality Assurance experience is a plus).
• Ability to travel independently to patient homes throughout the Queens borough.
• Excellent interpersonal and communication skills.
