Position Overview

We are seeking a compassionate and clinically strong Registered Nurse (RN) to support patient enrollment, engagement,

and ongoing clinical monitoring within our RPM and CCM programs. This role combines clinical nursing care, patient

education, care coordination, and technology-enabled health monitoring.

Key Responsibilities

Patient Enrollment & Program Onboarding

• Enroll eligible patients into RPM and CCM programs in coordination with physicians and care teams.
• Educate patients on program benefits, expectations, and device usage.
• Assist with consent documentation and onboarding compliance requirements.
• Support patient activation during initial program enrollment.

Clinical Monitoring & Patient Care

• Monitor patient health data from RPM devices (vitals, trends, alerts).
• Conduct routine patient outreach calls and follow-ups.
• Provide health coaching, chronic disease education, and self-management guidance.
• Escalate clinical concerns appropriately to providers or medical leadership.

Care Coordination

• Collaborate with physicians, behavioral health clinicians, and care coordinators.
• Maintain accurate clinical documentation aligned with CMS billing requirements.
• Support continuity of care across multiple clinical programs.

Patient Engagement & Support

• Help patients understand their health conditions and treatment plans.
• Encourage adherence to medications, lifestyle recommendations, and monitoring protocols.
• Build trust with patients through empathetic communication.

Compliance & Quality Assurance

• Maintain HIPAA compliance and documentation accuracy.
• Follow CMS RPM/CCM program guidelines and billing requirements.
• Participate in quality improvement initiatives.

Required Qualifications

• Active Registered Nurse (RN) license (California preferred).
• Minimum 2+ years clinical nursing experience.
• Strong patient communication and education skills.
• Experience with electronic health records (EHR).
• Comfort using healthcare technology and remote monitoring tools.

Preferred Qualifications (Plus, Not Required)

• Experience with Hoag or other major healthcare systems.
• RPM, CCM, telehealth, or care coordination experience.
• Chronic disease management experience.
• Medicare population experience.
