We are seeking a Part time Registered Nurse to join our outpatient family medicine practice. This position offers an opportunity to work in a collaborative environment focused on delivering high-quality patient care.

We provide comprehensive medical, dental, vision and behavioral health services for uninsured adults in Virginia in a faith-based setting that honors the name of Jesus Christ. We do not take insurance or any federal funding.
Registered Nurse - Position summary
Provides direct nursing care and chronic care self-management education in a primary/specialty outpatient clinic during normal business hours. Performs clinical assessments and nurse-led visits, supports providers through care coordination and follow-up, documents in Epic, and collaborates with the interdisciplinary team to optimize chronic disease outcomes (with a focus on diabetes and hypertension).
Qualifications
- Active Registered Nurse (RN) license
- Outpatient clinic experience and chronic disease management experience preferred
- Proficiency with Epic and secure patient messaging (MyChart) preferred
Essential duties and responsibilities
Direct patient care & clinical support
- Perform pre-visit and same day clinical intake: initial assessment, vital signs, medication reconciliation, point-of-care testing (POCT), and administration of standardized screening tools (e.g. PHQ-9, GAD-7, Mini-Mental).
- Administer injections and vaccines and assist providers with in-clinic procedures.
- Provide wound care, suture removal, PPD placement and interpretations, and monitor patient response to medication/treatment changes.
Nurse-led visits & chronic disease management
- Conduct nurse encounters including:
- Anticoagulation support: INR monitoring and Coumadin management per protocol/provider direction.
- Hypertension management: blood pressure monitoring, education, and follow-up.
- Diabetes management: review self blood glucose monitoring (SBGM), reinforce self-management skills, and support medication adjustment in collaboration with the PCP.
- Provide medication management support: reconciliation, patient education, and organization strategies for adherence.
Care coordination, results management & communication
- Track diagnostic testing and referrals; follow up on results and communicate recommendations/next steps to patients and providers, including medication and treatment changes.
- Communicate with patients by phone, in person, and via MyChart regarding refills, medication questions, lab/diagnostic results, procedures, and care plan concerns.
- Collaborate with PCP, LMAP, and pharmacist to process refills, maintain anaccurate medication list, and assist patients with obtaining prescriptions at outside pharmacies.
- Prepare letters of medical necessity for patient assistance programs when needed.
Documentation & clinic operations (Epic)
- Document all care, encounters, and care coordination activities in Epic; manage and respond to clinical messages; retrieve and route pertinent clinical data (labs, imaging, consult notes) to providers.
- Assist with chart preparation, chart build, and review of patients for medical eligibility as assigned.
Team collaboration, quality, and program support
- Participate in clinical and administrative meetings; collaborate on coordination of patient care services (e.g., CPAP, counseling, pastoral care) and contribute to development of clinic policies, procedures, guidelines, and grant related activities as assigned.
- Maintain exam room readiness by stocking exam rooms, nursing supply closet, and storage cabinets.
- Orient and supervise volunteer nurses.
- Collaborate with the Connected Care program coordinator to develop and implement remote monitoring programs for hypertension and diabetes.
Community engagement (as assigned)
- Support Development/Eligibility efforts by identifying appropriate patient testimonials and participating in fundraising and community awareness events to promote clinic services.
Work schedule & environment
Outpatient clinic, normal business hours
- Frequent patient interaction (in-person, phone, and electronic), standing/walking, and standard clinical procedures consistent with outpatient nursing practice.
