<Duties>
- Supervise and coordinate nursing staff during evening shifts, ensuring optimal patient care delivery across long-term care facilities.
- Oversee patient assessments, vital signs monitoring, medication administration, and medical documentation using electronic health record (EHR) systems.
- Ensure adherence to HIPAA regulations and infection control protocols to maintain patient confidentiality and safety.
- Collaborate with interdisciplinary teams including physicians, therapists, and case managers to develop and implement individualized care plans.
- Conduct staff evaluations, provide coaching and mentorship, and facilitate ongoing training on topics such as IV infusion, catheter care, ventilator management, and sterile techniques.
- Lead discharge planning efforts and coordinate with families for patients requiring dementia or Alzheimer's care, memory care services, or specialized senior care.
<Skills>
- Proven leadership experience in nurse management with supervising responsibilities in fast-paced environments such as hospitals or nursing homes.
- Strong understanding of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for procedures like spinal taps or cardiac catheterization.
- Skilled in patient assessment techniques including vital signs measurement, physical examinations, specimen collection/processing, phlebotomy and venipuncture.
- Ability to manage complex cases involving behavioral health issues, dementia care or working with individuals with disabilities across diverse age groups including pediatrics and neonatal ventilator management.
- Excellent communication skills for triage assessments, health coaching sessions, discharge instructions, and team collaboration while maintaining compliance with HIPAA standards.
