Job Overview
We are seeking a dedicated and compassionate Registered Nurse or Licensed Practical Nurse to join our team on either the first or third shift. This role offers an exciting opportunity to provide high-quality patient care within a supportive environment. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to enhancing the well-being of our residents through comprehensive nursing practices.

Responsibilities

• Deliver direct patient care by assessing needs, providing direct care and implementing appropriate interventions.
• Administer medications and treatments accurately in accordance with physician orders and facility protocols.
• Monitor vital signs, perform physical examinations, and document patient progress using electronic health record (EHR) systems such as Point Click Care (PCC).
• Collaborate with interdisciplinary teams to coordinate care plans for residents with complex medical needs, including those requiring dialysis, IV infusion therapy, or wound management.
• Ensure compliance with HIPAA regulations and maintain accurate medical documentation for all patient interactions.
• Assist with discharge planning and coordinate follow-up care to support residents' transition from hospital or acute care settings.
• Practice aseptic techniques during procedures.
• Provide specialized care for residents with memory care needs.

Experience
Previous experience is preferred, but we will train a new graduate. Laboratory procedures and infection control standard knowledge is preferred. Additional qualifications include proficiency in EMR/EHR systems like Point Click Care and knowledge of medical terminology and physiology, and experience working with individuals with disabilities or complex chronic conditions such as dementia. Certification in basic life support (BLS) and skills in IV insertion, aseptic technique, and medication administration are essential for success in this role.
