Location: 6400 Hwy 90 W New Iberia, LA (Central Office)
Monday- Friday 8:00am - 4pm ( 1hour paid Lunch)
What You’ll Do
As a Full-Time Registered Nurse (RN), you will provide clinical oversight and coordination of healthcare services while supporting participants and staff across the Waiver program.
Clinical Oversight & Participant Care

Conduct initial participant visits in person, along with required annual visits and follow-ups after hospitalization.

Monitor participant health status and respond to changes in condition.

Coordinate care with physicians, hospitals, pharmacies, and other healthcare providers.

Serve as a clinical resource to staff and support participant health needs.
Medication Administration & Compliance

Oversee medication administration practices to ensure accuracy and compliance.

Review Medication Administration Records (MARs) and medication counts monthly.

Address medication errors, discrepancies, or concerns promptly.

Ensure all health-related services align with regulatory and organizational standards.
Training & Staff Support

Train and support DSPs on medication administration and health-related tasks.

Conduct and document annual competency assessments.

Provide ongoing guidance to ensure staff are confident and competent in their roles.
Care Coordination & Communication

Communicate with physicians, families, and interdisciplinary teams to support participant care.

Assist with coordination of medical appointments and follow-up care.

Ensure required consents are obtained, including sending forms or facilitating verbal consent as appropriate.
Documentation & Resource Oversight

Maintain accurate, timely clinical documentation.

Oversee ordering of medical supplies and medication refills.

Ensure compliance with all documentation, visit schedules, and training requirements.
Qualifications
What We’re Looking For

Active Louisiana RN license (required).

Minimum of one (1) year of professional nursing experience required.

Experience in community-based or IDD services preferred.

Strong clinical judgment, organization, and documentation skills.

Ability to train, support, and communicate effectively with staff.

A professional, respectful approach to working with participants, families, and team members.
