The Momentum Project is seeking a Relief Host Home Provider to offer short-term care and support for individuals with developmental disabilities in your own home.

This flexible, paid opportunity allows you to provide a safe, supportive environment while helping individuals maintain stability and routine.

Responsibilities

• Provide a safe, welcoming, and structured home environment
• Support daily living activities such as meals, hygiene, and routines
• Supervise and ensure safety at all times
• Follow individualized support and behavior plans
• Encourage independence and community involvement
• Communicate with the support team and report any concerns
• Document care and significant events as required

Requirements

• Must provide care in your own home
• Safe and stable living environment
• Dependable, flexible, and able to provide relief as needed
• Experience with caregiving or developmental disabilities preferred
• Ability to pass background checks
• Complete 30+ hours of training
