Remote Engagement/Intake Nurse (LPN)

About the Role

Aviator Health/Oxbow Health is seeking an experienced Remote Engagement / Intake Nurse (LPN) to serve as the first clinical point of contact for incoming patients covered by Medicare and Medicare Advantage. In this role, you will complete initial clinical intake assessments, engage patients and caregivers, verify eligibility, and coordinate a smooth transition into care coordination services. This position is essential to delivering a positive patient experience while ensuring regulatory and documentation standards are met.

What You’ll Do

  • Conduct initial clinical intake assessments for new Medicare and Medicare Advantage patients
  • Engage patients and caregivers through clear, compassionate education and communication
  • Collect and document medical history, medications, functional status, and social determinants of health
  • Identify clinical risks, needs, and red flags and escalate appropriately
  • Verify coverage and eligibility
  • Accurately document assessments and interactions in the electronic health record (EHR)
  • Coordinate handoffs and transitions to providers and health advocates.
  • Support compliance with CMS and Medicare Advantage plan guidelines
  • Contribute to patient satisfaction, quality measures, and care outcomes

What You Bring

  • Active, unrestricted LPN license (do we require compact?)
  • Graduation from an accredited nursing program
  • Minimum 2 years of clinical nursing experience
  • Experience working with Medicare and/or Medicare Advantage populations
  • Strong clinical assessment, documentation, and communication skills
  • Comfort with telephonic and/or virtual patient engagement

Preferred Experience

  • Intake, care management, population health, transitional care, or home health
  • Familiarity with CMS regulations and Medicare Advantage requirements
  • Experience using EHR and care management systems
  • Knowledge of social determinants of health and community resources

Schedule

12-hour days, Saturday and Sunday plus one 8-hour weekday shift

Pay: $22.00 - $23.00 per hour
