This position is ideal for an NP with strong leadership skills who thrives in a dynamic startup environment and is deeply committed to caring for vulnerable populations. We are hiring for both a Nurse Practitioner and a Lead Nurse Practitioner position (with higher compensation for the Lead role).

Requirements

• 1+ years of experience as a licensed Nurse Practitioner
• Active/clear Nurse Practitioner license to practice in the State of Arizona
• Active/Clear DEA
• 1+ years of experience working in a rehab setting /rehabilitation facility centers
• Strong knowledge in HIPAA compliance as it pertains to telehealth
  
  

Key Responsibilities

• Serve as the operational lead for the interdisciplinary care team
• Lead care coordination efforts for complex Medicaid populations
• Practice autonomously with prescribing authority
• Manage care delivery primarily through telehealth and remote coordination
• Coordinate care across multiple providers, including primary care physicians and health plan case managers
• Support proactive care management, including risk stratification and early detection of health declines
• Provide 24/7 clinical access to help patients avoid unnecessary Emergency Room visits
• Support patients during hospitalizations and post-discharge transitions
• Help build and shape a new, innovative value-based care model
  
  

Schedule and Compensation

• Schedule: Primarily a virtual care model with collaborative interdisciplinary teams
• Compensation: The base salary can range from $130,000 to $175,000, depending on experience and whether the role is a Leader position
