Job Title: Collaborating Physician (Remote RPM/CCM Program)

About Us:
We operate a Medicare-approved Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) service. We partner with clinics, home care agencies, and facilities to provide full-service patient monitoring and care coordination.

Position:
We are seeking a licensed physician (MD or DO) in NY or NJ to serve as the collaborating provider for our RPM and CCM patients.

Our team handles patient enrollment, monitoring, and documentation. You simply review monthly summaries and approve billing.

Responsibilities:

Review and sign patient care plans and reports

Collaborate with our care coordination team

Bill Medicare under your NPI for RPM/CCM services (codes 99457, 99458, 99490, etc.)

Maintain compliance with CMS regulations

Requirements:

Active NY or NJ medical license

Medicare credentialed

Experience in Primary Care, Internal Medicine, Family Practice, or Geriatrics preferred

Open to remote collaboration

Compensation:
Flat per-patient fee or revenue share based on volume (flexible).
