Job Summary

The Reproductive Health Nurse will provide clinical oversight and curriculum-based education to the pregnant and parenting population and other youth of age in the MATCH Program of youth in Baltimore City Department of Social Services (BCDSS) Foster Care.

Duties And Responsibilities

Administration

  • Attends weekly team meetings to review case assignments and transfers.
  • Participate in all meetings as required by BCDSS.
  • Participate in Multi-disciplinary team meetings, inter-agency meetings and conferences.


Case Management

  • Coordinate holistic healthcare for children in the MATCH Program by partnering with the Baltimore City Department of Social Services (BCDSS) foster care program, foster parents, biological parents, managed care organizations, and healthcare providers.
  • Ensure the medical and mental health care needs of children in foster care are being met in a timely manner.
  • Assess children who are at risk for potential medical and mental health complications and monitor closely.
  • At registration for children sheltered with BCDSS, identify all immediate medical needs to begin health care coordination.
  • Review and assess all initial medical documentation from all mandated foster care exams.
  • Make recommendations in determining the appropriate continuing care team
  • Work with Program Manager to conduct weekly case transfer meetings.
  • Provide recommendations, resources, and education to caregivers and child welfare workers.
  • Assist foster care families with navigating the Health Choice system in establishing and maintaining a medical home for children in the BCDSS foster care system.
  • Prioritize care and contacts based on urgency- including ability to multi-task with accuracy.
  • Document case management assessments, medications, treatment plans, activities, and resolutions in BCDSS and the HCAM Program database.
  • Identify youth at risk for pregnancy related complications, parenting difficulties, and monitor closely.


Outreach & Education

  • Provide curriculum based reproductive health education, resources and recommendations to all identified youth in out of home placement with Baltimore City Department of Social Services to maintain wellness
  • Participate in staff, community, professional and inter-agency meetings and conferences.


Human Resources

  • Submit timesheet timely and accurately.
  • Complete all training required by HealthCare Access Maryland and funders.


Additional duties and responsibilities:

  • Additional duties as assigned.
  • May be required to work past normal working hours based on business needs.


QUALIFICATIONS:

Education, Experience, Certifications and Licensures:

  • A Bachelor’s degree in nursing from an accredited college/university (required)
  • Current licensure as a Registered Nurse in the State of Maryland. (required)
  • Relevant clinical experience in Reproductive/Maternal/Child Nursing, preferably in a public health/community setting


Knowledge, Skills & Abilities

  • Strong computer literacy skills, specifically with MS Word, Excel, Outlook and SharePoint (365) as well as Google Suite.
  • Organizational skills with the ability to meet a demanding workload
  • Detail oriented to complete requirements of files and contract compliance.
  • Experience in public speaking
  • Sensitivity to cultural and socioeconomic characteristics of population served.
  • Excellent oral and written interpersonal and communication skills required
  • Problem solving skills
  • Proven ability to work efficiently and meet deadlines


Compensation

In compliance with Maryland's wage transparency requirements, the compensation for this role has been included in the posting.
