Job Overview
We are seeking a dedicated and compassionate Resident Care Coordinator to join our team in providing exceptional care and support to our residents. This vital role involves coordinating personalized care plans, supervising caregiving staff, and ensuring that residents' needs are met with dignity and respect. The ideal candidate will have a strong background in senior care, nursing home experience, or working with individuals with developmental disabilities, along with excellent leadership and management skills. As a Resident Care Coordinator, you will play a key role in fostering a safe, engaging, and supportive environment for our residents while managing daily operations efficiently. Must have a CNA or HCA.
Responsibilities
- Develop, implement, and regularly update comprehensive care plans tailored to each resident’s unique needs, including Alzheimer’s and dementia care.
- Supervise and mentor caregiving staff to ensure high-quality service delivery aligned with regulatory standards and best practices.
- Oversee medication administration procedures, ensuring accuracy and compliance with safety protocols.
- Coordinate with healthcare professionals, families, and community resources to support residents’ physical, emotional, and social well-being.
- Manage daily operations related to resident care services, including scheduling, staffing levels, and resource allocation.
- Monitor residents’ progress and adjust care plans accordingly to promote optimal health outcomes.
- Maintain detailed documentation of care activities, incident reports, and regulatory compliance records.
Qualifications
- Proven experience in assisted living facilities, nursing homes, or long-term care environments.
- Strong background in senior care, memory care (including Alzheimer’s and dementia), and working with individuals with developmental disabilities.
- Demonstrated supervisory or management experience within a healthcare or home & community care setting.
- Knowledge of care planning processes, medication administration, and regulatory standards for long-term care facilities.
- Excellent leadership skills with the ability to motivate teams and foster a collaborative work environment.
- Must have an HCA or CNA
- Financial acumen related to budgeting and resource management within healthcare operations.

Pay: $24.00 - $26.00 per hour
