If you are an LPN or RN looking for a leadership position, join View Ridge Care as a Rehab Resident Care Manager (RN or LPN). The primary purpose of your job position is to plan, organize, develop, advocate and direct the overall care management of the rehabilitation patients in our care. Along with the other nurse managers, the Director of Nursing, rehab department, and social services you will work diligently to assure that the medical, emotional and social needs of the resident are met/ maintained on an individual basis.

Position Highlights:

Report to the Director of Nursing

Oversee patient care on assigned patients; medical, rehab progress and customer services.

Complete case management on all rehab patients - identify barriers to discharge and coordinate care to resolve them if applicable or assist to put alternatives in place.

Participate in rehab care management meeting. Communicate patient progress toward discharge, and follow-through on unresolved issues.

Coordinate and communicate with managed care organizations and other insurance providers as applicable- assist when needed in obtaining authorizations and extensions.

Coordinate and lead care conferences on a schedule and as needed.

Oversee management program for assigned patients to include but not limited to: ensuring tracking is in place and complete, attending management meeting, completing care plans.

Assist DNS and SDC in the management of nurses and CNAs that are providing care on your unit.

Pay: $48.00 - $52.00 per hour
