Job Description:
We are seeking a compassionate and reliable Care Coordinator to provide in-home support services to individuals with developmental disabilities living in an independent setting in Aurora and Denver area. This role is ideal for someone who is passionate about making a difference in the lives of others and is looking for flexible, meaningful work during the week.
As a care coordinator, you will play a vital role in supporting individuals with disabilities, mental health conditions, or other special needs in a variety of settings. Your primary responsibility will be to provide personalized care and support to help individuals lead fulfilling and independent lives.
Responsibilities:
- Provide person-centered care and support tailored to the client’s unique needs
- Assist with daily living activities, including hygiene, meal preparation, and light housekeeping
- Support the client in maintaining routines and accessing community activities, if applicable
- Document daily services provided in compliance with DD waiver requirements
- Communicate effectively with agency staff and supervisors
- Provide transportation to individuals as needed.
- Implement and document individual goals and medical tracking electronically.
- Offer emotional support and companionship to individuals, helping to build trust and foster positive relationships.
- Manage challenging behaviors and develop strategies for behavior management.
- Maintain accurate records and communicate effectively with team members, families, and healthcare professionals.
Requirements:
- Experience working with individuals with intellectual and/or developmental disabilities preferred
- Knowledge of behavioral management techniques and strategies for supporting individuals with complex needs.
- Must be 21 years or older
- Must have a reliable personal vehicle and valid driver’s license
- Must be able to pass background checks and provide references
- CPR/First Aid certification and Medication Administration Certification (or willingness to obtain)
