Residential Case Manager

The Residential Case Manager is responsible for managing the full referral and intake process, ensuring timely access to residential SUD treatment, and supporting program census and enrollment goals. This role includes completing intake, assessment, and admission documentation, coordinating client admissions with internal clinical and medical teams, delivering educational groups and life-skills classes, and providing ongoing case management support throughout treatment. The ideal candidate is organized, self-motivated, solution-focused, and committed to supporting men in early recovery using trauma-informed and person-centered practices.

Duties & Responsibilities

  • Receive, review, and process all referrals for residential SUD services
  • Maintain professional communication and coordination with referral partners, including hospitals, detox centers, treatment providers, courts, reentry programs, and community agencies
  • Conduct intake interviews and complete all required admission, consent, and enrollment paperwork
  • Coordinate client admissions with the Program Supervisor, SUD Counselor, and Nurse
  • Assist with diagnostic and clinical assessments as needed
  • Facilitate structured client education and recovery-based classes as assigned
  • Transport and accompany clients to and from external appointments when required
  • Complete clinical documentation in the EHR, including: assessments, treatment plans, progress notes, referral, discharge, and care coordination records.
  • Submit all documentation within 24 hours of service activity
  • Maintain 60% caseload productivity
  • Participate in the on-call rotation as scheduled
  • Engage in clinical supervision as assigned with a properly credentialed behavioral health professional

Support program enrollment goals and maintain client engagement to promote full census when clinically appropriate

Education & Experience

  • Bachelor’s Degree in Social Work or related behavioral health field is preferred
  • Must be eligible for at least one of the following credentials: CDCA (Chemical Dependency Counselor Assistant) and QMHS (Qualified Mental Health Specialist)
  • Previous experience in addiction treatment, behavioral health, residential programming, or community case management strongly preferred

Job Type: Full-time

Pay: From $16.00 per hour
