JOB TITLE: RN Admissions Nurse / QA Nurse Back-up

This is a "Road Warrior / Help Where Needed" position.
- This hybrid role requires a mix of new client assessments and care coordination, admissions visits, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as serving as QA Nurse back-up/support as needed.

Willingness to perform occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews is desired (not required).

At least 1 year of home health agency (or equivalent) experience is required.

Must have experience with OASIS and NON-OASIS assessments.

1 or more years of experience in home health care, wound care, lab draws, and/or case management / nurse supervision is a plus.
SERVICE AREA: Southeast Ohio Region with 2-hour radius including Jackson County, Vinton County, Gallia County, Pike County, Athens County, and Ross County [with optional coverage of Scioto and Adams Counties if willing]

Typical Daily Travel is between 0-90 minutes from home, but willingness to travel up to a 2-hour radius is required.

Round-trip Mileage Reimbursement is provided.
JOB LOCATION OPTIONS:
- Norwalk, OH in Huron County is centrally located to serve all cases within this service area, however qualified applicants residing in or adjacent to any of the defined counties will be considered so long as travel requirements are fulfilled.

HCN will expand this role into Full-Time (choice of hourly or salary) for the right candidate willing to travel up to a 2-hour radius.

Part-Time RNCMs receive hourly compensation (paid weekly, on Fridays).

If PRN Nurse: with hours and shifts varying week to week, including some weeks having low or no hours at all.

Weekend, Overtime, Respite & PRN services may be requested occasionally but are not required.

- This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel).

Compensation:

Hourly Wage: $35 - $40 per hour
- Salary: $73,000 - $85,000 per year + PTO (pay is negotiable based on travel radius and agreed-upon average services per week)
Compensation is based on experience, specializations and credentials, geographies served, and overall fit.
Job Summary
As an RN Admissions Nurse / QA Nurse Back-up, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.

Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.

You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
Reports to: Regional or Clinical Administrator / Director of Nursing
Responsibilities/Requirements of an RN Admissions Nurse / QA Nurse Back-up include:

RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.

RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.

RN to Document/Communicate/Coordinate patient status/change in condition to care team.

RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.

Willing to travel surrounding counties.

Minimum of one (1) years of home care experience required.
Qualifications

Graduate of an accredited School of Nursing.

Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.

CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).

At least 1 year of home health agency (or equivalent) experience is required.

Must have experience with OASIS and NON-OASIS assessments.

1 or more years of experience in home health care, wound care, lab draws, and/or case management / nurse supervision is a plus.

2+ years of home health experience highly preferred.
- Willingness to travel throughout all stated counties / 2-hour service radius to care for clients is required.

Strong background in nursing, patient care, and confidentiality.

Strong attendance and dependability track record with previous employers.
