Join Our Team as an RN Care Coordinator / Education Coordinator!

Are you a detail-oriented RN who enjoys supporting clinical excellence, education, and regulatory compliance? We are seeking a dedicated RN Care Coordinator / Education Coordinator to join our team at Colonial Manor. In this role, you will play a key part in supporting the MDS process, ensuring accurate clinical documentation, and helping educate and mentor nursing staff to maintain high standards of care.

Position Details

• Location: Colonial Manor

• Position Type: Full-time (64 hours per pay period)

• Compensation: Hourly

• Travel: Periodic

• Reports To: Director of Nursing

What You’ll Do

• Provide onsite clinical support to assist with MDS assessment data collection and documentation accuracy

• Support the MDS Coordinator with resident assessments and care plan development

• Provide education and training for nursing staff on MDS documentation and MatrixCare processes

• Conduct documentation audits and education to ensure compliance and improve charting quality

• Assist with care conferences and interdisciplinary care planning

• Monitor and report changes in resident condition to the interdisciplinary team

• Collaborate with therapy and other departments to support accurate assessments and reimbursement

• Participate in discharge planning and care coordination

• Support new admissions and post-hospital returns when needed

• Stay current on Medicare, documentation, and regulatory requirements

What We’re Looking For

• Active Registered Nurse (RN) license

• Strong knowledge of MDS processes, documentation, and regulatory compliance

• Experience with clinical documentation systems (MatrixCare preferred)

• Strong communication and collaboration skills

• Passion for staff education, quality improvement, and resident advocacy

• Ability to work effectively with interdisciplinary teams
