RN Care Manager – Longwood at Oakmont

Verona, PA – Full-time; Salaried, Exempt; M-F schedule

Starting Salary: $66,700 - $85,000 annually, depending on experience

The RN Care Manager is the first point of contact between care management services and the independent living residents at Longwood at Oakmont. This position interfaces with independent living residents to explain care management services, how care needs can best be met with services provided on the campus, develop an agreed upon service plan, and assign the residents to a care manager. The role assists residents to gain an understanding of what services should be brought into the home to maintain independent living status and facilitates the residents’ acceptance of services by our preferred providers.


Care management involves deliberately organizing resident care activities and sharing information among all the service providers concerned with a resident's care to achieve safer and successful independent living. This means that the residents’ needs and preferences are known ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care.

Requirements for the RN Care Manager:

  • Holds an RN license in good standing from the State of PA.
  • Has 4 years of relevant experience; experience with geriatrics and/or dementia care is preferred.
  • Demonstrated experience with case management, nursing assessment, and clinical skills are required.
  • Ability to work well with people and show enthusiasm and motivation.
  • Self-directed and able to set priorities; excellent time management skills.
  • Knowledge of Microsoft Office programs and medical records software.
  • Desire to contribute to our compassionate, resident-focused culture and effectively interact with residents, resident families, and team members.
