Job Description

JOB SUMMARY

The RN Case Manager is a collaborative process which assess, plans, implements, coordinates, monitors and evaluates options and services to meet the patient’s needs through communication and available resources to promote quality cost-effective outcomes.

ESSENTIAL JOB RESPONSIBILITIES

  • Gather all relevant data from chart review and patient interview.
  • Objectively identify barriers, clarify/determine realistic goals and seek potential alternatives.
  • Work with patient/family/physician/healthcare tam to develop a treatment/discharge plan to promote positive patient outcomes and cost effective care.
  • Initiate and implement plan modifications secondary to treatment response through continuous monitoring and communication with the healthcare team.
  • Actively promote communication between healthcare team members, patient/family, and physicians.
  • Monitor and promote appropriate use of resources, patient status and level of care.
  • Communicate with third party payors to ensure payment and maximize benefits.
  • Support and educate patient/family, physician, and healthcare team on available resources and regulations.
  • Represent the best interest of the patient and hospital by advocating for necessary funding, treatment alternatives, timelines and coordination of health services and re-evaluation of progress and goals.
  • Regular and reliable attendance is an essential function of this position

JOB QUALIFICATIONS

Required:

  • Registered Nurse
  • Current Nursing License in the State of Kansas
  • One year Hospital Case Management and/or Managed Care and Discharge Planning experience

Preferred :

  • Bachelor of Science in Nursing
  • Case Management Certification
  • Three years Medical/Surgical experience in a clinical setting
  • Experience utilizing InterQual
