Your Impact Zone: As an RN Case Manager, you'll travel throughout the Akron and Solon Aurora areas, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals –wherever they feel most comfortable.

Required Qualifications:

• Active RN License

• Valid Driver's License

• 1+ years hospice experience

Preferred Qualifications:

• BLS Certification

• Hospice and Palliative Medicine background
