Amber Hospice is a compassionate and dedicated hospice care provider committed to delivering exceptional end-of-life care to patients and their families. We are seeking an experienced and compassionate Hospice RN Case Manager (RNCM) to join our team and make a meaningful impact.

Job Summary:

The Hospice RN Case Manager is responsible for coordinating and providing high-quality hospice care to patients in their homes or facilities. This role involves assessing patient needs, developing individualized care plans, and working collaboratively with an interdisciplinary team to ensure comfort, dignity, and quality of life.

Key Responsibilities:

  • Provide skilled nursing care to hospice patients, including pain and symptom management.
  • Conduct comprehensive assessments and develop personalized care plans.
  • Educate patients, families, and caregivers on the hospice philosophy and care process.
  • Coordinate with physicians, social workers, chaplains, and other team members to ensure holistic care.
  • Monitor patient conditions and adjust care plans as needed.
  • Facilitate communication between the patient, family, and care team.
  • Document all patient interactions and care provided in compliance with regulatory guidelines.
  • Participate in on-call rotation and respond to patient needs after hours, as required.
  • Support patients and families emotionally and psychologically during the end-of-life process.

Qualifications:

  • Registered Nurse (RN) license in [State] (required).
  • Minimum 1 year of hospice, palliative care, or home health experience (preferred).
  • Strong knowledge of hospice philosophy, end-of-life care, and symptom management.
  • Ability to work independently and as part of a multidisciplinary team.
  • Excellent communication and interpersonal skills.
  • Valid driver’s license and reliable transportation for patient visits.
