Job Title: RN Discharge Planner
Duties and Responsibilities:
- Responsible for performing needed assessments on patients and delivering Medicare messages by staying up to date with CMS guidelines.
- Utilizes community resources to ensure that the patients have adequate follow-up care.
- Develop relationships with outside agencies and make referrals as appropriate.
- Works closely with the Utilization Nurse and will share responsibilities, such as reviewing patients for medical necessity and coordination of patient care.
- Coordinate and facilitate family meetings to discuss care.
- Documents discharge planning and social services in an ongoing manner.
- Demonstrates the ability to be flexible, organized, and function under stressful situations.
- Consults other departments as appropriate to collaborate on patient care and performance improvement activities.
- Treats patients and families with respect and dignity.
- Communicate with family members and caretakers regarding the needs of the patient and anticipated plans.
- Works well independently and within a multi-faceted team environment.
- Ability to relate well with multiple stakeholders, including physicians, nurses, allied health professionals, outside agencies, and patients.
- Accurately determines the type of assistance/setting/resources necessary for the patient/family and provides appropriate resources/assistance list of facilities.
- Maintains a good working relationship both within the department and with other departments.
- Documentation meets current standards and policies.
Professional Requirements:
- Ensures that appearance and personal conduct are professional at all times.
- Excellent attendance record.
- Wears appropriate clothing for job functions. Wears an ID badge.
- Works at maintaining a good rapport and a cooperative working relationship with physicians, administration, and staff.
- Represents the organization positively and professionally in the community.
- Maintains patient confidentiality at all times.
- Complies with all organizational policies regarding ethical business practices.
Regulatory Requirements:
- Current Registered Nurse licensure in the State of Utah.
- Is a motivated, independent individual who can organize workload so that all functions are completed appropriately.
- Working knowledge of the criteria for Medicare, Medicaid, HMO, and private insurance coverage.
- Experience in providing patient care.
