RN Clinical Care Coordinator

Maple Woods Manor is seeking highly motivated Registered Nurses to fill our RN Clinical Care Coordinator position. This position is integral in optimizing our residents' ability to meet their fullest potential. The CCC is responsible for the MDS assessment and management of the residents through the review of medical records, quality indicator reports, care plans and physical assessments. Prepares, provides and documents medication and treatment administration whenever necessary.

Wages:

  • $44.95/hour - $51.95/hour (based on experience)
  • *Including $0.85/hour MI wage pass through until 09.30.25
  • ADDITIONAL $1/ per hour added for BSN candidates

Essential Functions of the position include, but are not limited to:

  • Interview residents, resident representatives and hers to complete minimum data set assessment
  • Collect all data necessary to complete minimum data set forms and documentation
  • Make appropriate referrals to other health care professionals
  • Participate and conduct interdisciplinary meetings
  • Supervise and participate in the overall care of the residents
  • Oversee clinical coordinator, review labs, assist physician with rounds
  • Participate in QAPI
  • Follow established standards of nursing practices and implement policies & procedures.
  • Supervise and Lead all direct resident care.
  • Provide supervision to Nursing Assistants.
  • Responsible for compliance with federal, state and local regulations.

Qualified candidates will have:

  • An unencumbered Registered Nurse License
  • A minimum of one year of experience guiding the delivery of care in a Long-Term Care environment
  • The ability to demonstrate analytical and problem-solving skills
  • Have an Infection Preventionist Certificate, or willing to obtain one.

  • Must have the flexibility to support staff on multiple shifts
  • The ability to work in a long term care facility as required by regulations
