We are seeking a dynamic and experienced RN Clinical Coordinator to lead and coordinate patient care activities within our healthcare facility. Responsibilities
- Oversee daily clinical operations, ensuring compliance with hospital policies, state and federal regulations, including HIPAA and NCQA standards.
- Coordinate patient assessments, vital signs monitoring, and medical documentation to support accurate diagnosis and treatment planning.
- Supervise nursing staff and collaborate with multidisciplinary teams to deliver exceptional patient care across various settings such as medical-surgical units, ICU, pediatrics, geriatrics, behavioral health, or home care.
- Facilitate discharge planning, medication administration, IV infusion procedures, catheterization care, and other specialized nursing interventions like ventilator management or tube feeding.
- Utilize electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth for documentation, order entry, and data management.
- Conduct triage assessments and coordinate diagnostic evaluations including laboratory testing or medical imaging when necessary.
- Lead quality improvement initiatives by monitoring clinical outcomes and ensuring adherence to safety protocols like infection control and aseptic techniques.
Requirements
- Valid RN license with current state licensure; additional certifications such as Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), or Pediatric Advanced Life Support (PALS) preferred.
- Minimum 2+ years of recent clinical coordination, long-term care (LTC), home health, or case management experience preferred.
- Direct experience handling insurance authorizations, eligibility verification, and payer communication required.
- Experience working within long-term care (LTC), skilled nursing, rehabilitation, home health, or managed care environments.
- Strong clinical skills including patient assessment, medication administration, IV insertion/injection techniques, sterile processing procedures, and specimen collection/processing.
- Ability to work effectively with diverse populations including individuals with developmental disabilities, dementia or memory care needs; experience in geriatrics or pediatric settings is highly valued.
