What You'll Do In This Role:
Bring your clinical expertise to a role where it truly matters. As an RN Clinical Coordinator at LifeCircles PACE, you’ll provide participant-centered nursing care across clinic, home, and facility settings—using PACE-focused assessments and advanced clinical judgment to manage complex needs and support individualized care goals.
Partnering with the Interdisciplinary Team, you’ll coordinate holistic, high-quality care that improves outcomes and brings the PACE model to life—helping older adults live with dignity, independence, and confidence.
Facilitating Participant Integration
- Facilitate participant integration into LifeCircles’ healthcare system, including conducting medication reviews, administering immunizations, and monitoring both acute and chronic medical conditions.
- Conduct initial and periodic assessments in participant homes, acute and long-term care facilities, the PACE center, and other community settings.
- Identify participant needs, set priorities, interpret diagnostic results, and perform advanced nursing assessments using critical thinking.
- Assess clinical and Activities of Daily Living (ADL)/Instrumental Activities of Daily Living (IADL) support needs in the home environment.
- Promptly report unusual findings to the primary care provider, supervisor, and/or Interdisciplinary Team (IDT).
Planning Care
- Actively participate in IDT meetings and contribute to the development of the participant care plan across all care settings.
- Collaborate with Home Care Coordinators to develop and monitor care plans for home health aides, ensuring clinical and ADL needs are met.
Care Coordination
- Administer treatments, procedures, medications, and other prescribed care in accordance with patient care standards.
- Oversee and monitor participant medications, demonstrating understanding of age-related characteristics and needs.
- Provide clear explanations of nursing procedures to participants, offer discharge teaching, and coordinate transitions between care settings.
- Recognize emergency situations and initiate advanced therapy independently.
- Coordinate patient care with other healthcare personnel, evaluate instituted care measures, and ensure the 24-hour delivery of care services.
- Work with Home Care Coordinators and other team members to coordinate Durable Medical Equipment (DME) and medical supplies (e.g., oxygen, hospital beds, incontinence supplies, wound care supplies, ostomy supplies, tube feeding supplies).
Evaluation/Documentation
- Accurately document all care in the participant's medical records as per policy, including care delivered, variations from the care plan, and observations of potential or impending issues.
- Record changes or deviations in participant care and provide rationale for decisions made.
Leadership
- Delegate nursing care to appropriate personnel, ensuring effective communication using the 7 C's of communication: Clear, Concise, Concrete, Correct, Coherent, Complete, and Courteous.
- Actively support and participate in Quality Assurance and Performance Improvement (QAPI) processes to promote safe working conditions and appropriate delivery of participant care.
Participate as part of the on-call nursing staff, adhering to on-call policies and procedures.

Education
- Diploma or degree from an accredited School of Nursing or University.
- Current Registered Nurse (RN) license to practice in the State of Michigan.
Experience
- Minimum of one year of experience working with the frail elderly or long-term care population.
- Previous experience in home care or long-term care is preferred.
Skills & Competencies
- Strong clinical assessment and nursing skills with the ability to manage complex medical situations.
- Ability to work independently and as part of an interdisciplinary team (IDT).
- Knowledge of PACE-specific care, interventions, and regulatory standards is a plus.
- Excellent communication and interpersonal skills, with the ability to work effectively with participants, families, and the healthcare team.
