Job Summary

We are seeking a Registered Nurse (RN) Clinical Liaison to drive referral growth and support patient transitions from acute care settings to home infusion services. This role combines clinical expertise with business development and relationship management, working closely with physicians, discharge planners, case managers, and payers to identify appropriate patients and coordinate seamless transitions of care.

Job Responsibilities

  • Generate referrals and grow territory business through relationship development and account management
  • Build and maintain strong partnerships with physicians, discharge planners, case managers, and healthcare facilities
  • Identify patients who may benefit from home infusion services and coordinate care transitions
  • Collaborate with healthcare teams to develop patient care plans and support discharge planning efforts
  • Educate referral sources on available infusion services and clinical capabilities
  • Serve as a clinical resource between referral sources, patients, and internal care teams
  • Manage territory activity, referral opportunities, and follow-up efforts
  • Support patient onboarding and ensure a smooth transition from hospital to home-based care

Qualifications

  • Current and unrestricted Registered Nurse (RN) license required
  • Minimum 2 years of infusion healthcare experience required
  • Sales, business development, healthcare marketing, or referral development experience preferred
  • Experience working with physicians, discharge planners, case managers, and healthcare facilities
  • Strong relationship-building, communication, and presentation skills
  • Ability to work independently while managing a territory
  • Strong organizational and problem-solving abilities
