Job Title: Homecare Nurse
Job Summary:
The Homecare Nurse provides comprehensive and compassionate nursing care to patients in their own homes. This role involves delivering patient-centered care by performing clinical assessments and offering education to patients and their families. The Homecare Nurse works closely with interdisciplinary teams to develop and implement individualized care plans that promote the health, safety, and well-being of patients in the home environment.
Key Responsibilities:
· Patient Assessment and Care Planning:
o Conduct initial and ongoing assessments to evaluate patients’ physical, emotional, and social needs.
o Develop and implement personalized care plans based on assessment findings and physician directives.
o Monitor patient progress and adapt care plans as needed.
· Patient and Family Education:
o Educate patients and families on disease management, medication adherence, and self-care techniques.
o Provide guidance on maintaining a safe and healthy home environment.
o Assist caregivers with techniques to support the patient's ongoing care and comfort.
· Documentation and Reporting:
o Maintain accurate and thorough documentation of patient care, including assessments, interventions, and progress notes.
o Report changes in patient conditions to the supervising nurse or physician promptly.
o Submit required reports and documentation in a timely and accurate manner.
· Collaboration and Communication:
o Communicate effectively with physicians, therapists, social workers, and other members of the healthcare team.
o Attend case conferences and interdisciplinary meetings to discuss patient progress and care coordination.
o Coordinate with home health aides and other support personnel to ensure continuity of care.
Qualifications:
· Education:
o Associate or Bachelor’s Degree in Nursing (ADN or BSN) from an accredited nursing program.
o Current and valid Registered Nurse (RN) license in the state of practice(New York).
· Experience:
o Minimum of 1-2 years of hospital or acute care preferred.
o Home Care Experience preferred.
o Experience with chronic disease management and geriatric patients is preferred.
· Skills:
o Strong clinical assessment, critical thinking, and problem-solving abilities.
o Exceptional communication and interpersonal skills.
o Ability to work independently and manage multiple patients effectively.
· Certifications:
o Basic Life Support (BLS) certification required.
Working Conditions:
· Travel to patient homes within the assigned area.
· Maintain infection control protocols and safety procedures during all visits.

Pay: $140.00 - $150.00 per hour
