Family hospice is looking to add to our existing team of leaders for SC! This position would primarily report to our Myrtle Beach office on a consistent basis, but require travel to surround offices and potentially partnering facilities.

As the Hospice Clinical Manager, you will guide and support your clinical team to care for our patients during their hospice journey when they need you most.

Qualifications

An enthusiasm for providing the best possible care to patients and their families.

A South Carolina Registered Nurse license in good standing

3 years of experience in hospice with a minimum of 1 year in hospice leadership preferred

A valid driver’s license

Current CPR certification

Excellent observation, verbal, and written communication skills

RESPONSIBILITIES (including but not limited to):

· Assigns clinical staff to patients and families based on patient need, including chaplain, social workers, volunteers, dietary counselors, therapists for PT/ST/OT, and bereavement coordinators at the direction of the Executive Director.

· Coordinates hospice services for each patient and family.

· Directs staff in performance of their duties including admission, discharge, transfer, revocation, and provision of service to patients.

· Ensures that interdisciplinary care is provided.

· Oversees orders and approves DME and nursing supplies.

· Ensures complaints are addressed timely and within standards of hospice with complaint log up to date.

· Participates in conflict and complaint management and resolution.

· Maintains effective channels of communication including integration or technology, as applicable.

· Ensures appropriate staff supervision during all service hours.

· Monitors service utilization to ensure delivery of comprehensive care.

· Ensures standards of ethical business and clinical practice are maintained.

· Makes field visits to evaluate appropriateness, determine eligibility for services and present hospice program.

· Communicates assessment findings to Attending Physician and initiates treatment plan per physician’s orders.

· Keeps Executive Director informed of daily activities and issues. Proposes solutions to all issues.

· Maintains an effective relationship with management, staff, and general public.

· Follows all company policies and procedures, state and federal laws and regulations and charged with reporting violations to the appropriate supervisor.

· Maintains confidentiality of company information.

· Demonstrates honesty and integrity at all times.

· Able to respond to change productively and handle additional projects as assigned.

· Able to carry out the duties and responsibilities of this job (with or without reasonable accommodation) without posing a specific or current risk of substantial harm to the health and safety of self and others.

· Assists with maintaining compliance with federal, state, and agency regulations and accreditation agency standards.

· Performs other duties as assigned by Executive Director
