Responsibilities

• Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
• Utilize EHR systems for accurate documentation and tracking of patient progress.
• Collaborate with interdisciplinary teams to enhance patient care in long-term and assisted living settings.
• Provide education and support to patients and families regarding health management and care procedures.

Job Type: Full-time

Pay: $29.00 - $32.00 per hour
