Compensation

• Start of Care (SOC) – Medicare: $120
• Recertification / Resumption / Discharge: $85 (work submitted within 3 days of visit)
• IV Visit: $85 (work submitted within 3 days of visit)
• Routine RN Visit: $55

Responsibilities

Home health in office position.

Head of Staff in the office to monitor compliance.

You have good problem solving skills.

You Ensure that there are processes in place to monitor and measure all activities related to performance improvement with our nursing staff.

Coordinate activities related to clinical record review for data utilization Compiled , trends and reports quality data in the following areas:- patient care , safety , risk management, infection control , outcomes and customer satisfaction .

As a member of the patient’s Grievance Committee collaborates with management personal in follow up of force RN’s expressed by patients, families and physicians.

Coordinate educates and implementats monitoring activities for safety risk management and infection control

Coordinate mandatory inservice program on OSHA regulations for safety and infection control Act as resource to personnel in identifying safety and risk management issues for patient and personnel .

Reviews all incident reports for completeness , risk, standard of care determination and appropriate follow up . Maintain a log for trending and make recommendations related to trends identified.

Collaborate with DON to provide a process th promote and ensure employee orientation, competency and inservice education programs.

Monitor organization compliance with regulatory and accreditation standards. Acts as a clinical resource to personnel by maintaining updated nursing knowledge and skills , assist with patient visits as needed , assist with administrative call and clinical calls .

Provides assistance to community education team in meeting community education needs by acting as the representative of the organization at community meetings, civic and professional organization .

Promotes self - growth and collegial relationships with others in the home care industry.

Establish a process model for improvement and provided education and training to organization personnel on the performance improvement plan as well as team development.

Prospectively reviews all oasis assessments to ensure appropriateness , completeness, and compliance with federal and state regulations and organization policy . Utilizes oasis variation or alert report when reviewing oasis data Ensure appropriate ICD 10 coding and sequencing as it relates to the patients medical condition , including any comorbidities .

LVN Starting rates, depending on your practical experience.
