Anova Care is currently seeking compassionate and experienced Registered Nurses (RN), Physical Therapists (PT), and Occupational Therapists (OT) to join our expanding Home Health team in Northern Colorado.

Position Overview

As part of our Home Health team, you will provide skilled care and support to patients in their homes while working collaboratively with an interdisciplinary care team.

This role is ideal for clinicians who value flexibility, autonomy, and meaningful patient relationships.

Responsibilities

• Provide skilled nursing or therapy care to home health patients
• Assess patient needs and develop appropriate care plans
• Educate patients and families on treatment and care management
• Collaborate with physicians and interdisciplinary care teams
• Maintain accurate and timely documentation

Qualifications

RN

• Active RN license in Colorado
• Home Health or Hospice experience preferred

PT

• Licensed Physical Therapist in Colorado
• Home Health experience preferred

OT

• Licensed Occupational Therapist in Colorado
• Home Health experience preferred
