Position Summary

We are looking for an experienced and detail-oriented Registered Nurse Assessment Coordinator (RNAC) to oversee and manage the MDS assessment process at our facility. In this key leadership role, you will ensure accurate, timely completion of resident assessments in full compliance with federal and state regulations, directly impacting the quality of care and reimbursement outcomes for our residents.

Key Responsibilities

  • Coordinate and complete Minimum Data Set (MDS) assessments accurately and on schedule
  • Ensure compliance with RAI (Resident Assessment Instrument) guidelines and CMS regulations
  • Collaborate with nursing, therapy, social services, and dietary teams to gather comprehensive assessment data
  • Lead care planning meetings and ensure individualized, goal-directed care plans are in place
  • Monitor assessment reference dates (ARDs) and maintain the facility assessment calendar
  • Identify and communicate changes in resident condition that trigger new assessments
  • Ensure accurate coding to support appropriate Medicare and Medicaid reimbursement (PDPM)
  • Educate and mentor nursing staff on MDS-related documentation and compliance
  • Maintain current knowledge of regulatory updates, CMS changes, and industry best practices
  • Participate in quality assurance and performance improvement (QAPI) initiatives

Qualifications & Requirements

  • Current, valid Registered Nurse (RN) license in Pennsylvania — required
  • Minimum 1–2 years of MDS coordination or assessment experience — preferred
  • Strong working knowledge of PDPM, RAI Manual, and CMS compliance requirements
  • Excellent clinical assessment and critical thinking skills
  • Proficiency in Point Click Care (PCC) or similar EHR/MDS software — preferred
  • Strong attention to detail with the ability to meet deadlines
  • Effective communication and collaboration skills across all departments
  • RAC-CT certification (AANAC) — a plus
