Title: RN/NP/PA Wellness & Aesthetics Provider - Peptide, Hormone, Regenerative Medical and Aesthetics (Health & Wellness)

Reports To: Medical Leadership (Dr. Pollei) + Sr. Director of Operations

Form MD is an end-to-end concierge plastic surgery, aesthetics/medical spa, and wellness practice that consists of a rockstar team & culture caring for amazing patients & delivering natural outcomes.

We are seeking a part- to full-time NP/PA/RN Wellness Educator to lead patient education, facilitate treatment protocols, oversee clinical workflows, and ensure seamless integration of both in-person and virtual care. This role is ideal for a versatile provider who is confident delivering comprehensive care across both wellness and aesthetic services, and who can effectively manage and treat any patient placed on their schedule.

The position includes driving patient care plans for our subscription-based peptide, hormone, and wellness protocols, while also performing in-person services such as medical weight loss, IV and vitamin therapy, hyperbaric oxygen therapy, and aesthetic treatments. Candidates should have experience in both wellness and aesthetics, with the ability to flex between services based on patient needs and clinic demand.

You’ll play a key role in shaping how our brand is experienced—in-office and throughout the community—with a strong focus on patient experience, outcomes, and education. We are looking for a provider who is proactive, adaptable, and committed to delivering high-quality, comprehensive care across all service lines.

For background, we moved into a beautiful new office space three years ago with subsequent space expansion totaling 12 patient care rooms. An adjacent accredited Surgical Center buildout is in the planning phase. Our three Form MD brands provide 1) Plastic Surgery, 2) Medical Spa / Aesthetics, and 3) Wellness treatments. Our surgical practice includes a wide breadth of cosmetic & reconstructive facial as well as breast & body plastic surgical procedures. The medical spa entity with its Aesthetic NP/RN and Esthetician team provides all toxin, filler, lasers, modalities and treatments needed for aesthetic rejuvenation. Our performance (wellness) entity with its team provides functional wellness, peptide protocol & hormone therapy, medical weight loss, IV, vitamin, Hyperbaric oxygen, etc. and is also growing with a similar concierge and patient focused ethos.

Key Responsibilities

1. Elevated Patient Experience & Clinical Care (In-Person + Virtual):

• Deliver a concierge-level patient experience by educating and guiding patients through personalized wellness and aesthetic treatment plans, including peptide therapy, hormone optimization, and advanced aesthetic services.
• Provide expert consultations that thoughtfully blend clinical insight with an aesthetic eye, ensuring patients feel confident, informed, and cared for at every step of their journey.
• Perform a full spectrum of services within scope, including IV and vitamin therapy, hyperbaric oxygen therapy, medical weight loss, as well as aesthetic treatments such as neurotoxins, dermal fillers, and laser procedures.
• Seamlessly manage and treat any patient on the schedule, with the ability to transition between wellness and aesthetic services based on individual patient needs and goals.
• Maintain impeccable documentation and oversee each patient’s journey with a high level of attention to detail and follow-through.
• Act as a trusted clinical partner alongside Dr. Pollei to curate and execute individualized, results-driven care plans.

2. Seamless Execution & White-Glove Follow-Through:

• Ensure flawless execution of both in-clinic and virtual care plans, maintaining the highest standards of safety, quality, and patient satisfaction.
• Proactively monitor patient progress, anticipating needs and delivering timely follow-up, adjustments, and ongoing support.
• Collaborate cross-functionally with clinical, pharmacy, and operations teams to create a seamless, elevated patient experience across all touchpoints.
• Partner with Leadership and Operations to ensure alignment across scheduling, technology, patient flow, and overall brand experience.

3. Clinical Excellence & Innovation:

• Serve as a clinical expert across both wellness and aesthetic services, confidently managing complex cases with professionalism and discretion.
• Stay at the forefront of advancements in regenerative medicine, hormone therapy, and aesthetic treatments (injectables, lasers, skin health), bringing best-in-class offerings to patients.
• Continuously refine and elevate care delivery, identifying opportunities to enhance both outcomes and the overall patient experience.

4. Patient Advocacy & Relationship Building:

• Cultivate long-term relationships with patients through trust, attentiveness, and a genuine commitment to their goals and results.
• Deliver an exceptional level of service by addressing patient needs with empathy, responsiveness, and professionalism.
• Leverage digital platforms and communication tools to provide ongoing, high-touch engagement and support beyond the clinic visit.

Qualifications

Required:

• Current Physician Assistant (PA) or Nurse Practitioner (NP) license in the state of California.
• Minimum 3-5 years of clinical experience, with a strong background in peptide protocols, medical weight loss, hormone optimization and/or integrative medicine. Minimum 2 years of medspa aesthetics experience.
• Expertise or strong interest in leading-edge peptide therapy and virtual care delivery.
• Proficiency in virtual care platforms and telehealth technology.
• Exceptional communication, organizational, and problem-solving skills.
• 3 years of clinical metric & tracking / KPI implementation and utilization
• Sound understanding of regulatory requirements, healthcare standards, and ethical guidelines.
• Demonstrate the ability to manage, lead, and hold team members accountable
• Experience with strategic growth objective identification and execution
• Strong analytical and problem-solving capabilities to identify areas for improvement and implement effective solutions.

Preferred:

• Specialized training in integrative medicine, functional medicine, hormone therapy, peptide therapy, or integrative health.
• Experience in patient education, clinical training, and virtual health environments
