Rural Community Nurse

Position Overview

The Rural Health Outreach Nurse supports access to care and population health for rural and underserved communities, including Amish and Plain populations served by Primary Health Network. Working in collaboration with providers, nurses, Community Health Workers, and community partners, this role serves as a liaison between patients and the healthcare system to help individuals overcome barriers related to transportation, communication, health literacy, technology access, and care coordination.

The Rural Health Outreach Nurse promotes health and wellness through culturally responsive outreach, patient education, advocacy, telehealth support, and care coordination activities designed to improve access, continuity of care, and health outcomes.

This role emphasizes relationship-building and community engagement to help address health disparities and improve equitable access to healthcare services in rural communities.

Compensation

Position starts at $20.00, however, PHN recognizes experience, education and site location when determining wage.

Location and Hours of Position

Home site: PHN Operations Department 63 Pitt Street, Sharon Pa 16146

Hours: Monday – Friday 8:30am – 4:30pm

This role requires flexibility, as work location and hours may vary in response to program needs and outreach activities.

NO WEEKENDS

Qualifications

Licensed Practical Nurse (LPN): graduate of Licensed Practical Nurse program and licensed or eligible to take state boards.

  • Infection Control and strong clinical knowledge
  • Administering Medication
  • Previous venipuncture and injection experience preferred
  • Previous experience in calling prescriptions under a providers direction
  • Computer proficient and EMR experience
